16. Kwetsbaarheid

Samenvatting

Veel ouderen hebben te maken met (chronische) aandoeningen, functionele beperkingen en/of multimorbiditeit. De term kwetsbaarheid wordt in dit kader vaak gebruikt. Toch bestaat er geen eenduidigheid over wat men precies onder deze term verstaat. De benadering van kwetsbaarheid als een medisch syndroom gaat ervan uit dat er een beperkt aantal factoren of componenten is dat ten grondslag ligt aan een specifiek fysiologisch proces. Op basis hiervan kwam Linda Fried met het model van de ‘frailty cycle’. Anderen zien kwetsbaarheid als ‘opeenstapeling van factoren’, ook wel ‘deficit accumulation’ genoemd. Tussen deze twee uitersten, liggen nog tal van andere visies, waaronder het kwetsbaarheidsmodel volgens Gobbens. In dit hoofdstuk worden de verschillende definities van kwetsbaarheid besproken, de diverse aspecten die bij kwetsbaarheid een rol spelen en de verschillende manieren hoe dit gemeten kan worden in de dagelijkse praktijk. Veelgebruikte meetinstrumenten zijn: Fried-criteria, Frailty Index accumulation of deficits, Groningen Frailty Indicator (GFI), Tilburg Frailty Indicator (TFI) en de Evaluative Frailty Index for Physical activity (EFIP).
