Stamceltransplantatie voor aangeboren stofwisselingsziekten

Samenvatting

Voor een geselecteerde groep aangeboren stofwisselingsziekten (ASZ) kan een stamceltransplantatie (SCT) de progressie van klinische verschijnselen voorkomen. Dit geldt met name voor specifieke lysosomale stapelingsziekten en peroxisomale stoornissen en alleen indien de SCT vroeg in het leven wordt uitgevoerd, voordat hersenschade is opgetreden. Voor de ziekte van Hurler, X-gebonden adrenoleukodystrofie en de ziekte van Krabbe zijn inmiddels veelbelovende resultaten gepubliceerd. Echter, het succes van SCT bij ASZ werd in belangrijke mate beperkt door zowel hoge percentages donorafstoting en mortaliteit gerelateerd aan de procedure alsook het tijdsinterval tussen diagnose en behandeling door moeilijkheden bij het vinden van een geschikte stamceldonor. Een verbetering van de transplantatietechnieken, het gebruik van onverwant navelstrengbloed als stamcelbron en de mogelijkheid tot gebruik van adjuvante intraveneuze enzymvervangingstherapie kunnen het succespercentage van de procedure, en daarmee ook de prognose van deze ernstig aangedane kinderen, verbeteren. Een goed functionerende wereldwijde registratie, waarbij de uitkomst (op langere termijn) van SCT bij ASZ nauwkeurig wordt vastgelegd is daarbij van essentieel belang.
