36. Nekpijn (aspecifieke; zonder radiculopathie)

Samenvatting beleid

Richt anamnese en lichamelijk onderzoek op het uitsluiten van de specifieke oorzaken van de nekpijn. Leg bij aspecifieke nekpijn uit dat dit geen specifieke oorzaak heeft, vaak vanzelf overgaat en soms een wisselend en langdurig beloop kent. Adviseer de informatie te lezen op www.​thuisarts.​nl/​nekpijn/​ik-heb-nekpijn. Adviseer bij klachten korter dan 2 weken en veel pijn eventueel eenvoudige pijnstillers en stimuleer om zo veel mogelijk gewoon te blijven bewegen en pijnstillers alleen te nemen als de pijn te hinderlijk wordt. Dit kan worden aangevuld met houdings- en bewegingsadviezen. Overweeg bij klachten langer dan 2 weken te verwijzen naar een fysiotherapeut of manueel therapeut voor een activerende en functiegerichte behandeling. Bij chronische pijn en grote psychologische belasting kan cognitieve gedragstherapie overwogen worden. Bij zeer gelokaliseerde chronische pijn kan een lidocaïne-injectie in de pijnpunten overwogen worden. Bij therapieresistentie kan acupunctuur worden overwogen.
