Achtergrond Wat kan de huisarts doen voor patiënten met nekpijn? Dat is niet altijd duidelijk. Wij onderzochten het beleid van de huisarts in het eerste contact met een patiënt bij een nieuwe episode van nekpijn.
Doel Beschrijven welke diagnostische en therapeutische interventies de huisarts doet bij acute nekpijn, en wat de zelfzorg van de patiënt inhoudt.
Methode Wij volgden 187 patiënten met acute aspecifieke nekpijn uit 29 huisartsenpraktijken steeds een jaar lang. Bij hun eerste bezoek aan de huisarts mochten zij niet langer dan 6 weken pijn gehad hebben. Elke patiënt vulde bij inclusie en vervolgens na 6, 12, 26 en 52 weken een vragenlijst in, en gaf tevens op een zevenpuntsschaal aan in hoeverre hij zich hersteld voelde.
Resultaten De huisartsen verwezen op de baseline 51% van de patiënten naar de fysiotherapeut en schreven aan 42% medicijnen voor, voornamelijk NSAID’s (56%) of spierontspannende middelen (20%). De meest gegeven adviezen waren: afwachten (23%), het ‘verbeteren van de houding’ (22%), actief blijven (22%) en rust nemen (18%). De zelfzorg van de deelnemers bestond voornamelijk uit warmteapplicatie (79%) en oefeningen (57%). Verwijzingen naar een medisch specialist en aanvragen van röntgenfoto’s kwamen weinig voor.
Conclusie De behandeling van de huisarts volgt meestal één van twee min of meer gelijkwaardige sporen: wachten op een gunstig beloop met ondersteuning van medicijnen, of verwijzen naar een fysiotherapeut.
