Allergie op de kinderleeftijd – een rol voor primaire preventie

Samenvatting

De laatste tientallen jaren is er een toename te zien in de prevalentie van allergische aandoeningen zoals astma, hooikoorts en eczeem bij kinderen. Tot dusver is de behandeling vooral gericht op klachtenbestrijding. Primaire preventie van allergische aandoeningen, dus het voorkomen van de ziekte voordat er sensibilisatie en symptomen optreden, is een relatief nieuw onderzoeksgebied. In dit artikel wordt het onderzoek besproken dat zich richt op primaire preventie. Er wordt onderscheid gemaakt tussen interventies in de antenatale en de postnatale periode. Antenataal bestaat primaire preventie uit het vermijden van contact met allergenen. De effectiviteit hiervan staat echter niet vast. Postnataal zijn de mogelijkheden tot primaire preventie uitgebreider. Positieve effecten worden gezien bij het toedienen van probiotica. Onderzoeken naar het effect van toediening van Lactobacillus gg laten hoopvolle resultaten zien met betrekking tot het verminderen van atopisch eczeem. Uit dieronderzoek blijkt dat ook oligonucleotidetherapie een rol zou kunnen spelen bij de primaire preventie van allergie. Tegenstrijdige resultaten worden gezien bij het toepassen van borstvoeding, het vermijden van allergenen en bcg-vaccinatie. Prospectieve studies zijn nodig om het effect van deze interventies definitief op hun waarde te schatten.
