Samenvatting beleid

Indien iemand het spreekuur bezoekt met de klacht verstopping is het belangrijk eerst anamnestisch en door lichamelijk onderzoek organische en medicamenteuze oorzaken uit te sluiten. Inventariseer daarna eet- en leefgewoonten die invloed hebben op de ontlasting en vraag naar zelfmedicatie (laxantia). Geef aansluitend voorlichting en advies. Bij de medicamenteuze behandeling van functionele obstipatie gaat de voorkeur uit naar macrogol (met of zonder elektrolyten) of lactulose vanwege de osmotische werking, de klinische ervaring en het gebruiksgemak. Bij onvoldoende effect komt een contactlaxans (bisacodyl of sennosiden A+B) in aanmerking. Kies bij ernstige klachten voor rectale medicatie ter lediging van het rectum. Eventueel kan bij therapieresistente gevallen prucalopride worden voorgeschreven.
