Vergelijking tussen individueelgerichte en gezinsgerichte behandelingen voor internaliserende, externaliserende en gezinssymptomen bij Latino-jongeren

Samenvatting

Er zijn gedegen, aselecte trials nodig waarin veelbelovende cultuurgerichte behandelingen voor Latino-jongeren en gezinnen kunnen worden getest. Deze studie draagt bij aan de kennis hierover, namelijk door de werkzaamheid van een cultuursensitieve en flexibele gezinstherapie voor adolescenten (Culturally Informed and Flexible Family Treatment for Adolescents [CIFFTA]) te vergelijken met een individueel gerichte reguliere behandeling (Individually Oriented Treatment-As-Usual [ITAU]); een vergelijking op de vermindering van het vermogen om Latino-jongeren en gezinnen voor therapie te behouden (therapiebehoud), vermindering van internaliserende en externaliserende symptomen van het kind en op verbetering van het gezinsfunctioneren. CIFFTA gebruikt een adaptieve/flexibele aanpak voor individuele en gezinstherapie en biedt psycho-educatieve modules aan die zijn toegesneden op de unieke klinische en culturele kenmerken van het gezin. Tweehonderd Latino-adolescenten tussen 11 en 14 jaar oud voltooiden een baseline beoordeling en werden aselect toegewezen aan CIFFTA of ITAU, om na 16 weken interventie nogmaals beoordeeld te worden. De resultaten laten zien dat CIFFTA een significant hogere retentie (83%) had dan ITAU (71%), OR = 2,05, p = 0,036. Jongeren in beide condities lieten significante afnames zien in het door de jongeren en ouders gerapporteerde internaliserende en externaliserende gedrag, en tussen beide condities waren er geen verschillen in verandering. Ouders in CIFFTA meldden significant grotere afnames in gezinsconflict, d = 0,038, p = 0,025 dan in ITAU. In CIFFTA lieten kinderen van minder geaccultureerde Latino-ouders meer verbetering zien dan de kinderen van meer geaccultureerde ouders. In ITAU gold het omgekeerde, daarin meldden kinderen van meer geaccultureerde ouders een grotere verbetering. Dit bewijs voor de invloed van CIFFTA op therapiebehoud, op gezinsconflict en voor het differentiële effect afhankelijk van culturele waarden en gedragingen heeft belangrijke implicaties voor de psychologische behandeling en gezinstherapie in de Latino-gemeenschap.
