De beschrijvende psychiatrie volgens de criteria van de dsm-iv geeft geen wezenlijk inzicht in de oorzaken van psychische stoornissen. Een psychiatrische diagnose op zich leidt mede daarom meestal ook niet tot een functioneel geïntegreerde behandeling. Dat probleem doet zich ook en vooral voor bij de in de dsm-iv genoemde gedragsstoornis in de kinderjaren en de cluster-B-persoonlijkheidsstoornissen bij volwassenen. Behandeling van die stoornissen is over het algemeen dan ook niet erg succesvol. Wellicht is dat de aanleiding van het sinds de jaren tachtig van de vorige eeuw voor storend gedrag geldende uitgangspunt van no cure but control. Op grond van dat uitgangspunt staat niet genezing van de patiënt met een gedragsstoornis op de voorgrond maar controle van het over het algemeen vooral voor de omgeving storende gedrag. Zolang echter geen nadere functionele individuele diagnostiek is gedaan over de relatie tussen de vastgestelde psychiatrische diagnose en de vertoonde gedragingen moet de nodig geachte controle in principe van buiten worden opgelegd. Al of niet ondersteund door medicatie. Daarbij wordt echter ’ ten onrechte ’ geen onderscheid gemaakt tussen bijvoorbeeld impulsief gedrag met een gebrek aan controle en instrumenteel gedrag met juist een overmaat aan controle. …
