Samenvatting

In de spreekkamer bij de huisarts heeft de klacht pijn op de borst bij ongeveer 15 % van de patiënten een cardiale oorzaak. Vrouwen presenteren zich vaker dan mannen met symptomen en tekenen die wijzen op myocardiale ischemie zonder obstructief coronarialijden (INOCA en MINOCA). Pijn op de borst is het meest voorkomende symptoom van ischemische hartziekte, zowel bij mannen als bij vrouwen, maar komt bij vrouwen minder vaak voor. De bijkomende symptomen zoals de uitstraling en de vegetatieve symptomen verschillen tussen mannen en vrouwen. Ook in de risicofactoren is er verschil. De traditionele risicofactoren zoals hypertensie en roken zijn voor mannen en vrouwen gelijk, maar de impact van deze risicofactoren is bij vrouwen groter. Daarnaast hebben vrouwen ook vrouwspecifieke risicofactoren zoals een doorgemaakte pre-eclampsie. In de keuze van aanvullend onderzoek moet rekening gehouden worden met de grotere kans op ischemie zonder obstructie, die meer bij vrouwen voorkomt.
