16. Dyspnoe

Samenvatting

Kortademigheid of dyspnoe is een abnormale en onaangename gewaarwording van de ademhaling. De presentatie is zeer divers en de differentiaaldiagnose is breed, met respiratoire, cardiale, neurologische en psychische oorzaken. Leerboeken en richtlijnen besteden relatief weinig aandacht aan de verschillen tussen vrouwen en mannen, terwijl wetenschappelijke literatuur laat zien dat een gendersensitieve bril bij de zorgverlener wenselijk is. Vrouwen ervaren sneller dyspnoe en rapporteren dit vaker aan hun arts. Angst en somberheid als bijkomende klachten komen bij vrouwen vaker voor dan bij mannen. Ook bij diagnostiek en behandeling spelen verschillen tussen vrouwen en mannen een rol. Zo wordt bij mannen sneller een longfoto gemaakt of gekozen voor een doorverwijzing naar de specialist. Voor de belangrijkste diagnoses die horen bij de klacht dyspnoe, waaronder astma, COPD en hartfalen, bestaan duidelijke genderverschillen in prevalentie, presentatie en prognose.
