Samenvatting

Inhoud – 1 Inleiding – 2 Toepassingen – 3 Verschillen indicaties en resultaten na palliatieve radiotherapie – 4 Nieuwe geneesmiddelen, nieuwe bijwerkingen – Palliatieve behandelingen vragen om een multidisciplinaire aanpak, zodat er aandacht is voor alle facetten van het lijden, of dit nu op lichamelijk, psychosociaal of spiritueel gebied is. Het doel van palliatieve radiotherapie is het verbeteren van de kwaliteit van leven door het verminderen van klachten en symptomen ten gevolge van een lokale tumor of metastase. In de palliatieve fase is pijnbestrijding vaak het belangrijkste doel van de behandeling, maar ook compressie of bloeding kan hiervoor een aanleiding zijn. De ernst en de duur van de bijwerkingen van palliatieve bestraling zijn veel milder en korter dan die van curatieve bestraling, doordat een relatief lage stralendosis wordt gegeven in weinig fracties (hypofractionering). In dit hoofdstuk worden toepassingen van palliatieve radiotherapie van ongeneeslijke primaire tumoren en metastasen in verschillende organen en delen van het lichaam beschreven. De palliatieve behandeling voor de verschillende tumorlocaties is altijd afhankelijk van de conditie, prognose en wens van de patiënt.

Botmetastasen worden ongecompliceerd genoemd als ze niet hebben geresulteerd in of een hoge kans hebben op een fractuur en als ze geen neurologische symptomen veroorzaken door myelum- of caudacompressie.
