Ontwikkelingsbiologie van de long: pathogenese van bronchopulmonale dysplasie

Samenvatting

Bronchopulmonale dysplasie (bpd) werd in 1967 beschreven als een chronische longaandoening bij premature pasgeborenen met het respiratory-distress syndrome (rds) die langdurig waren beademd met hoge concentraties zuurstof. Nieuwe beademingstechnieken en de introductie van antenatale glucocorticoid en postnatale surfactantbehandeling hebben de perinatale longfunctie en de overlevingskansen van premature pasgeborenen sterk verbeterd. Parallel hieraan is ook het klinische en radiologische beeld van bpd veranderd. De ‘nieuwe’ bpd treft voornamelijk extreem prematuur pasgeborenen en wordt gekarakteriseerd door een verminderde alveolarisatie en vascularisatie in de perinatale long. Premature ratten blootgesteld aan hyperoxie ontwikkelen een longbeeld dat vergelijkbaar is met bpd. De initiële resultaten van longhistologie bij dit bpd-model worden beschreven, alsmede chemotaxis van neutrofielen en p38 mitogen-activated protein kinase (mapk)-activiteit in longweefsel. Deze laatste parameters suggereren een belangrijke causale rol voor inflammatie. Verder onderzoek met behulp van de dna-chiptechnologie is in gang gezet om de expressie van genen die een rol spelen in de ontwikkeling van bpd verder in kaart te brengen.
