Neutropenie bij een pasgeborene: differentiaaldiagnose en therapie

Samenvatting

Aan de hand van een casus worden de differentiaaldiagnose en de aanpak van persisterende neonatale neutropenie besproken. De oorzaken van neutropenie na de geboorte kunnen in drie groepen verdeeld worden: verminderde productie, toegenomen afbraak of combinatie van beide.
Eerst dienen bacteriële en virale infecties te worden uitgesloten. Familieanamnese, maternale anamnese en lichamelijk onderzoek van de patiënt kunnen aanleiding zijn voor aanvullende diagnostiek.
De beschreven patiënt had een allo-immuunneutropenie. Bij deze aandoening maakt de moeder na sensibilisatie IgG-antistoffen tegen neutrofielspecifieke antigenen. Deze zorgen na transplacentaire overdracht voor verhoogde afbraak van foetale granulocyten. Het is een weinig voorkomende, zelfbegrenzende aandoening, die gepaard kan gaan met ernstige, levensbedreigende infecties.
Bij deze patiënt werden de ernstige recidiverende infecties behandeld met antibiotica. Daarnaast werd de neutropenie bestreden met granulocytenkoloniestimulerende factor (g-csf). Deze casus toont aan dat deze in de oncologie veel gebruikte groeifactor een veilig en effectief middel is om bij ernstige infecties de allo-immuunneutropenie te behandelen.
