Een bijzondere casus: twee seksuele pijnproblemen bij één cliënt

Samenvatting

Over seksuele pijnklachten (dyspareunie) bij mannen is weinig literatuur te vinden en dit geldt zeker ook voor de dominantie van psychosomatische elementen in de etiologie ervan. De hier beschreven casus is bijzonder omdat de cliënt last had van twee van elkaar te onderscheiden typen genitale pijn: ten eerste pijn in de eikel van de penis, ontstaan na een periode waarin de voorhuid problematisch nauw was, en ten tweede pijn bij ejaculatie, gecombineerd met een aantal andere onderbuikspijnklachten die vallen binnen het ‘chronic pelvic pain syndrome’. Aan de hand van de verschillende klachten wordt een aantal aspecten van de etiologie van deze beide pijnproblemen uitgediept. Ik hoop de lezer met deze bijzondere casus te enthousiasmeren voor het psychosomatisch gedachtegoed, dat in de laatste jaren een comeback lijkt te beleven.

.Deze vorm van masturbatie werd beschreven door Sank (1998), die hulpverleners aanbeveelt om bij seksuele disfuncties altijd zorgvuldig te vragen naar de exacte manier van masturbatie.

Bij vrouwen die vanwege vaginisme in behandeling komen, horen we regelmatig een soortgelijk inzicht bij het bespreken van vroege tamponpogingen (en de vanzelfsprekendheid waarmee zij zich erbij neerlegden dat dat nu eenmaal niet mogelijk was).
