Samenvatting

In de toekomst moet voor een groeiend aantal thuiswonende ouderen kwalitatief goede zorg geleverd worden met minder financiële middelen. Op basis van het (inter)nationale wetenschappelijk bewijs kan geconcludeerd worden dat er op dit moment een gebrek is aan overtuigend effectieve interventies voor thuiswonende kwetsbare ouderen. In de toekomst zal vooral geïnvesteerd moeten worden in het bevorderen van zelfredzaamheid, samenwerking tussen zorg en welzijn, interprofessioneel samenwerken en een levensloopbestendige woon- en leefomgeving. Dit hoofdstuk start met een algemeen overzicht van interventies vanuit internationale wetenschappelijke literatuur en beschrijft eerstelijnsinterventies die recentelijk vanuit het Nationaal Programma Ouderenzorg (NPO) zijn uitgevoerd. Daarnaast wordt een overzicht gegeven van de stand van zaken rond effectiviteit van deze interventies en een indruk van recente ontwikkelingen in de zorg rond kwetsbare ouderen.
