In-vitrofertilisatie, ouderlijke subfertiliteit en de gezondheid en ontwikkeling van het nageslacht

Samenvatting

Ruim één op de tien paren kampt met subfertiliteit: hen lukt het niet om binnen een jaar zwanger te worden. Een toenemend aantal paren maakt gebruik van vruchtbaarheidsbehandelingen, zoals in-vitrofertilisatie (IVF). De gezondheid en ontwikkeling van IVF-kinderen is daarom van maatschappelijk belang. In dit artikel vatten we de literatuur over dit onderwerp samen. We maken hierbij ook gebruik van de gegevens van de Groningen ART (assisted reproductive technology) cohortstudie, een prospectieve, longitudinale follow-upstudie die de gezondheid en ontwikkeling van eenlingen van subfertiele paren, die al dan niet IVF ondergingen, in kaart brengt. Tot op heden werd de ontwikkeling van deze kinderen tot en met de leeftijd van vier jaar onderzocht. De belangrijkste conclusie is dat niet zozeer IVF-behandelingen, maar juist de onderliggende subfertiliteit van de ouders geassocieerd is met aangeboren afwijkingen en een minder optimale ontwikkeling van het zenuwstelsel. Een langere ‘tijd tot zwangerschap’ (TTZ), een maat voor de ernst van subfertiliteit, is hierbij bepalend. De resultaten benadrukken het belang van langdurige follow-up van kinderen van subfertiele ouders, die met of zonder IVF zwanger werden. Dit is vooral belangrijk in de huidige maatschappij waar de maternale leeftijd ten tijde van de conceptie, subfertiliteit en het toepassen van vruchtbaarheidsbehandelingen toenemen.
