14. Handelingsgebieden: wonen en zorgen

Samenvatting

De eigen woning is de plek bij uitstek voor het handelingsgebied zorgen: het slapen, het huishouden, de zorg voor jezelf en voor andere familieleden. Het beleid in de participatiesamenleving is dat iedereen de kans krijgt om aan de maatschappij deel te nemen, alleen de dagelijkse praktijk is vaak anders. Er komen steeds meer mogelijkheden voor mensen om hun specifieke woonwensen te realiseren, het aantal verschillende woonvormen neemt toe. De komende decennia ontstaat door de dubbele vergrijzing een toename van de vraag naar wonen en zorg. De steeds snellere technologische ontwikkelingen, met name op het gebied van ICT binnen de woning (domotica) en de zorg (zorg-op-afstand), zorgen voor een revolutionaire verandering in het handelen van de cliënt en zijn systeem, de zorgverlener, de zorgorganisaties. Zorgen bestaat uit een complexe interactie tussen de uitvoering van zelfzorgactiviteiten, activiteiten die noodzakelijk zijn voor het zich handhaven van het individu in het dagelijks en maatschappelijk leven, en activiteiten nodig om zorg te dragen voor anderen in de (directe) omgeving. Hierbij kan een onderscheid gemaakt worden tussen de basisactiviteiten van het dagelijks leven (BADL) en de complexere instrumentele activiteiten van het dagelijks leven (IADL). Het is daarbij essentieel om te weten wie iemand is (identiteit en onafhankelijkheid), wat zijn waarden en normen zijn (effectiviteit, motivatie, keuzes en betekenis) en in wat voor omgeving iemand leeft. De betrokkenheid van een persoon in het zorgen voor zichzelf en anderen staat centraal in de ergotherapie-interventie. Hoe deze factoren zich ontwikkelen kan men terugzien in de levensloop. Hierbij valt op dat het eerste deel van iemands leven in het teken staat van een groeiende zelfstandigheid, die dan lange tijd stabiel blijft en op latere leeftijd weer afneemt. Om de uitvoering van deze activiteiten in kaart te brengen zijn verschillende assessments beschikbaar. Het beschrijven van het dagelijks handelen kan helpen voor het bepalen van prioriteiten binnen dit handelingsgebied. Bij de ergotherapie-interventie kan er gewerkt worden op het niveau van de persoon en zijn systeem, de organisatie of populatie.
