8. Dissociatie

Samenvatting

Al ruim tweehonderd jaar proberen we het concept dissociatie te begrijpen, waarbij het met name wordt geassocieerd met psychotrauma en in het bijzonder seksueel misbruik. Dissociatie is echter een bekend fysiologisch mechanisme als reactie op stressvolle of traumatische ervaringen. In dit hoofdstuk bespreken we het brede spectrum van dissociatieve klachten en hoe deze zich kunnen presenteren in de praktijk, variërend van coping en habituatie tot uiteindelijk pathologische dissociatie. Tevens wordt het concept dissociatie vanuit verschillende modellen beschreven en wordt aan de hand van een casusbeschrijving de complexiteit van diagnostiek en behandeling geïllustreerd. Het voornaamste doel van dit hoofdstuk is inzicht te geven in het onderliggende mechanisme van dissociatie en hoe het lichaam kan spreken waar taal tekortschiet. Een pleidooi om de kennis vanuit het theoretische kader van dissociatie te vertalen naar de weerbarstige klinische praktijk, waarbij het desensitiseren van het stresssysteem en het ontkoppelen van een prikkel-responsreactie met dissociatieve verschijnselen centraal staat.
