3. De patiënt positief psychologisch benaderd

Abstract

Van fysiotherapeuten mag een visie op diagnostiek en therapie verwacht worden, zodanig dat zij een prognostisch gezondheidsprofiel van de patiënt kunnen maken. Een prognostisch gezondheidsprofiel is een beschrijving van het gezondheidsprobleem van de patiënt inclusief de relatie met onderliggende factoren en de onderlinge samenhang tussen die factoren. De toevoeging ‘prognostisch’ verwijst naar het benoemen van herstelbelemmerende en/of herstelbevorderende factoren die van invloed zijn op het beloop van het gezondheidsprobleem1. De herstelbelemmerende factoren krijgen in de fysiotherapie mime aandacht. Men kan daarbij denken aan een toestand van chronische stress of bijvoorbeeld aan disfunctionele ziekteopvattingen van de patiënt2. De introductie van herstelbevorderende factoren is nieuw en uitermate relevant. Een parallel met recente ontwikkelingen in de psychologie dringt zich hier op. De klinische psychologie werd vanaf zijn ontstaan gedomineerd door het medische model oftewel het ziekte-model. Problemen met het dagelijks leven, zoals relatieproblemen en seksuele moeilijkheden werden al snel geëtiketteerd als ‘stoornissen’. Stoornissen die met psychotherapie behandeld moesten worden.
