De diagnostische opbrengst van bloedonderzoek bij onbegrepen klachten

Bij 3 tot 39% van de patiënten die met klachten op het spreekuur komen, kan de huisarts na de anamnese en het lichamelijk onderzoek geen specifieke diagnose stellen. Men neemt aan dat de meeste onbegrepen klachten niet heel ernstig zijn en vanzelf weer overgaan. Soms echter blijken ze toch de (eerste) symptomen van een aandoening. Uit het VAMPIRE-onderzoek (VAgue Medical Problems In REsearch) blijkt dat de diagnostische opbrengst van later aangevraagd bloedonderzoek even groot is als die van direct aangevraagd bloedonderzoek, maar dat direct aanvragen in absolute termen meer kosten en meer fout-positieve uitslagen met zich meebrengt. Voorts blijkt dat het relatieve aantal fout-positieve testuitslagen fors toeneemt als men het aantal bloedtests uitbreidt. Deze bevindingen ondersteunen het advies in de teruggetrokken NHG-Standaard Bloedonderzoek om het aantal aangevraagde tests te beperken en bloedonderzoek in eerste instantie vier weken uit te stellen bij patiënten met onbegrepen moeheid.
