Inleiding Aspecifieke nekklachten zijn, na lage rugpijn en schouderklachten, de meest voorkomende klachten van het bewegingsapparaat. In Nederland is geen richtlijn beschikbaar voor de behandeling van aspecifieke nekklachten. Doel van dit literatuuronderzoek is in kaart te brengen welke onderzochte eerstelijnsbehandelingsmogelijkheden effectief zijn.
Methode Wij verrichtten een systematisch literatuuronderzoek naar de behandeling van aspecifieke nekpijn in de databanken Cinahl, EMBASE, MEDLINE en Cochrane. Twee onderzoekers includeerden onderzoeken onafhankelijk van elkaar en beoordeelden de artikelen onafhankelijk van elkaar op wetenschappelijke bewijskracht.
Resultaten Na selectie van 157 artikelen, bleven er 41 over die aan de inclusiecriteria voldeden. Over medicamenteuze behandeling bij nekpijn is slechts zeer weinig gepubliceerd. Paramedische behandelingen zijn uitgebreid bestudeerd, maar de kwaliteit van de onderzoeken laat te wensen over.
Beschouwing Acute nekpijn benadert men bij voorkeur expectatief of met analgetica. Voor subacute en chronische nekpijn is manuele therapie in combinatie met oefentherapie veruit de beste behandelingsoptie. Andere behandelingen worden besproken. Er is behoefte aan kwalitatief goed onderzoek naar de effecten van paramedische behandelingen.
