9. Cognitieve therapie bij posttraumatische stressstoornis

Samenvatting

Intens emotionerende negatieve gebeurtenissen leiden bij sommige mensen tot de posttraumatische stressstoornis (PTSS). De patiënt met PTSS verkeert in een langdurige toestand van prikkelbaarheid en wordt geplaagd door herbelevingen, negatieve stemming, angst en vermijding. Patiënten met PTSS lijden ook vaak aan gevoelens van schuld en schaamte, agressie, en verlies van eigenwaarde en vertrouwen in de wereld. Cognitieve therapie is een effectieve behandeling voor PTSS. Samengevat zijn er drie cognitieve factoren die de verwerking van een trauma belemmeren: (a) negatieve opvattingen over PTSS-symptomen, die leiden tot angst en vermijding, waardoor de verwerking stagneert; (b) vervorming van de traumatische gebeurtenis (assimilatie); (c) disfunctionele vervorming van de bestaande opvattingen (overaccommodatie) over zichzelf, andere mensen en de wereld. Het doel van cognitieve therapie bij PTSS is om deze factoren, indien aanwezig, op te heffen en de patiënt te helpen het trauma alsnog te verwerken.
