Biomarkers bij burn-outpatiënten

Samenvatting

Sinds Bradley (1969) het burn-outsyndroom voor het eerst beschreef, nu ruim 30 jaar geleden, heeft deze metafoor voor een toestand van psychologische uitputting sterk aan populariteit gewonnen. Recent onderzoek toont aan dat vier procent van de Nederlandse beroepsbevolking dusdanige symptomen van burn-out heeft dat ze zich eigenlijk onder professionele behandeling zouden moeten stellen (Houtman et al., 2000). Dit is een duidelijk teken dat burn-out een serieus probleem is, dat aangepakt moet worden.
Het stellen van de diagnose burn-out is echter niet altijd gemakkelijk. Lastig is dat er geen objectieve neurobiologische markers bekend zijn voor burn-out. Daarnaast komt de diagnose burn-out niet voor in de DSM-IV (American Psychiatric Association, 1994).
Burn-outpatiënten kunnen binnen de DSM-IV gediagnosticeerd worden op basis van het kernsymptoom van burn-out, langdurige en medisch onverklaarbare vermoeidheid, als lijdende aan een ongedifferentieerde somatoforme stoornis. In de ICD-10 (World Health Organization, 1992) komt een op burn-out gelijkend syndroom, het neurastene syndroom, wel voor als onafhankelijke diagnose, maar diagnostische criteria ontbreken.
