Acute gastro-enteritis bij kinderen

Implementatie van een richtlijn gebaseerd op snelle enterale rehydratie met ORS

Samenvatting

Achtergrond.

Sinds 10-15 jaar bestaan internationale, evidencebased richtlijnen voor acute gastro-enteritis op de kinderleeftijd. Gegevens over de haalbaarheid en het effect van implementatie van deze richtlijnen ontbreken.

Doel.

Evaluatie van het effect van implementatie van de richtlijn op opnameduur en heropnames.

Methode.

In de Isala klinieken wordt sinds 2009 een evidencebased richtlijn gehanteerd waarbij rehydratie met ORS 20 ml/kg/uur per sonde wordt toegediend gedurende 4 uur. Hierna blijven kinderen 2 uur ter observatie en gaan vervolgens met ontslag. Gegevens van kinderen opgenomen in verband met gastro-enteritis vóór invoeren van de richtlijn werden verzameld door middel van retrospectief statusonderzoek. De gegevens ná invoeren van de richtlijn werden verzameld in een prospectief cohortonderzoek.

Resultaten.

In het jaar voor implementatie werden 358 kinderen met een acute gastro-enteritis gezien, van wie 41% opgenomen; het jaar na implementatie 292 kinderen, van wie 43% opgenomen (verschil 1,7%, 95%-BI –5,9 tot 9,3; p= 0,653). Na exclusie van kinderen met een relevante voorgeschiedenis werden 143 kinderen voor implementatie geïncludeerd en 119 kinderen na implementatie. Van de kinderen is 83% volgens de richtlijn behandeld, vier kinderen zijn intraveneus gerehydreerd. De mediane opnameduur voor implementatie was 37 uur en na implementatie 16 uur (ratio van geometrisch gemiddelde 0,45; 95%-BI 0,37 tot 0,55; p < 0,001). Het aantal heropnames verschilde niet significant (voor 3% en na 4%; 95%-BI –0,9 tot 6,5; p= 0,534).

Conclusie.

Snelle enterale rehydratie met ORS gevolgd door ontslag bij kinderen met dehydratie bij gastro-enteritis is goed mogelijk. Implementatie van deze richtlijn leidt tot een halvering van de opnameduur, zonder toename van het aantal heropnames.
