5 Behandeling met een partiële gebitsprothese 2. Oorzaken en gevolgen van een gereduceerd occlusiesysteem

Samenvatting

● Het occlusiesysteem is onderdeel van het orofaciale systeem en bestaat uit de processus alveolaris maxillae en mandibulae en de al dan niet occluderende gebitselementen of prothese-elementen. De meest voorkomende oorzaken van het verlies van gebitselementen zijn ontoereikende mondverzorging en onvoldoende professionele mondzorg. Een gereduceerd occlusiesysteem kan worden geclassificeerd in termen van diastemen in het front en aantallen functionele paren gebitselementen in de zijdelingse delen, met als toevoeging een onderbroken dan wel een verkorte tandboog. Kenmerken van een gezond functioneel occlusiesysteem zijn afwezigheid van pathologie en aanwezigheid van toereikende functies zonder ongemakken. Gevolgen van reductie van een occlusiesysteem kunnen zijn: overbelasting, migraties van gebitselementen, mandibulaire instabiliteit en gestoorde esthetiek en kauwfunctie. Morfologische en functionele veranderingen kunnen als adaptatie aan gewijzigde situaties worden beschouwd of als pathologisch. De grens tussen adaptatie en pathologie is niet scherp te trekken. Als prothetische vervanging noodzakelijk is, dan is de trend om uitneembare constructies te vermijden.
