Wel of niet screenen op prostaatcarcinoom?

Abstract

Het nut van prostaatkankerscreening is (nog) twijfelachtiger dan het bevolkingsonderzoek op borst- of baarmoederhalskanker. Omdat kankerscreening per definitie overdiagnostiek en -behandeling betekent, zijn de consequenties daarvan ernstiger bij prostaatkankerscreening, gezien de nadelige effecten van radicale prostatectomie (incontinentie en/of impotentie). Omdat prostaatkankersterfte vooral op hoge leeftijd voorkomt, is de winst qua levensverwachting naar verhouding gering. Er wordt erg veel opportunistische screening door middel van psa-bepaling verricht, maar het nut daarvan is twijfelachtig en de kans op schade groot.
