Vroegtijdige interventieprogramma's voor prematuren: naar ontwikkelingsgerichte zorg

Samenvatting

In dit artikel wordt een overzicht gegeven van bestaande interventieprogramma's voor te vroeg geboren kinderen en hun ouders. Het Mother-Infant Transaction Program (mitp) leert in 7 sessies vlak voor en 4 sessies vlak na het ontslag van het kind uit het ziekenhuis aan moeders hoe hun kind reageert, wat het al kan en heeft daarmee een positief effect op de moeder-kind relatie. Het Newborn Individualized Developmental Care and Assessment Program (nidcap) is bedoeld om al op de nicu onnodige stress voor kind en ouders zo veel mogelijk te voorkomen. Met behulp van dit programma leren ouders, verpleegkundigen, paramedici en artsen goed kijken naar het gedrag van het kind en wordt een multidisciplinair zorgplan gemaakt waarin de omgeving en verzorging zo veel mogelijk worden aangepast aan de individuele behoeften van het kind. Kinderen die volgens deze methode worden verpleegd en behandeld, blijken minder lang opgenomen te blijven, korter beademing nodig te hebben, sneller te groeien en zich beter te ontwikkelen dan kinderen die niet volgens deze methode worden verpleegd. Gezien het verschil in cultuur en organisatie van de medische zorg tussen de Verenigde Staten en Nederland zijn deze interventieporgramma's niet zonder meer toepasbaar. Ouders zijn goed gemotiveerd en kunnen individueel worden benaderd. In het Sophia Kinderziekenhuis werd daarom als eerste stap het Vroegtijdig Interventieprogramma voor Prematuren (vip) als ouderprogramma ontwikkeld met het mitp en het nidcap als basis. Maar ook lange tijd na ontslag van hun kind uit het ziekenhuis hebben veel ouders nog ondersteuning nodig. De grote investering die men op de nicu doet kan alleen vruchten dragen als dit wordt aangevuld met ontwikkelingsgerichte zorg op korte en lange termijn.
