Vertrouw bij pneumonie op uw klinische blik

Achtergrond Bepaalde predictiemodellen stellen de huisarts in staat om een pneumonie te voorspellen aan de hand van klinische informatie. Daarmee kan het gebruik van dure diagnostiek beperkt worden. Deze modellen zijn nog niet gevalideerd in de huisartsgeneeskundige populaties.
Methode We deden een prospectief onderzoek tussen 15 november 1998 en 1 juni 2001 in Leiden en omgeving onder volwassen patiënten met een lageluchtweginfectie (LLI) naar de oorzaken van LLI. We verzamelden klinische gegevens en maakten een thoraxfoto. Daarnaast verrichtten we een literatuuronderzoek om predictiemodellen voor een pneumonie te vinden.
Resultaten Aan het onderzoek namen 129 patiënten deel: 26 met een pneumonie en 103 zonder pneumonie op de thoraxfoto. We pasten zes modellen toe. Alleen het model dat ook gebruikmaakt van de C-reactief proteïne (CRP) test toonde een significante oppervlakte onder de curve van 0,69 (95%-BI 0,58-0,80), met een positief voorspellende waarde van 47% (95%-BI 23-71%) en een negatief voorspellende waarde van 84% (95%-BI 77-91%). De voorafkansen voor een pneumonie en voor geen pneumonie waren respectievelijk 20 en 80%.
Conclusie Predictiemodellen gebaseerd op klinische informatie geven geen betrouwbare voorspelling van pneumonie. Aanvulling met een verhoogde CRP-waarde heeft beperkte waarde.
