41. Urine-incontinentie

Samenvatting

Bij alle patiënten met urine-incontinentie is het belangrijk om zorgvuldige diagnostiek te bedrijven, omdat de indeling consequenties kan hebben voor het beleid. In eerste instantie moet onderscheid gemaakt worden tussen ‘functionele incontinentie’, dat is ongewild urineverlies op basis van cognitieve of mobiliteitsstoornissen, en ‘echte’ urine-incontinentie op basis van urineweg- en bekkenbodemafwijkingen. Vervolgens is het onderscheid tussen stressincontinentie (inspanningsincontinentie) en urgency-incontinentie van belang. Daarnaast dient men inzicht te krijgen in de ernst en de gevolgen voor de patiënt. Het lichamelijk onderzoek is vooral bedoeld om de bekkenbodem te beoordelen en zeldzame oorzaken van urine-incontinentie uit te sluiten. Bij ernstige vormen van urgency- of stressincontinentie die niet reageren op conservatieve behandeling is verder onderzoek nodig. Bij reflex- en overloopincontinentie bij vrouwen en bij alle vormen van urine-incontinentie zonder duidelijke oorzaak bij mannen is het zinvol aanvullend onderzoek te verrichten naar een achterliggende oorzaak, zoals een neurologische aandoening, obstructie door prolaps of een vergrote prostaat.
