The effect of preoperative fasting on postoperative pain, nausea and vomiting in pediatric ambulatory tonsillectomy1)

Tonsillectomie en adenotonsillectomie zijn algemeen bekende, poliklinisch uitgevoerde operatieve ingrepen bij kinderen. Deze kinderen hebben postoperatief pijn en problemen met eten en drinken. In de eerste 24 uur na de behandeling is de pijn het hevigst, vooral bij het slikken.2) Bovendien is misselijkheid na tonsillectomie gebruikelijk en komt overgeven bij meer dan de helft van de kinderen voor.3-5) Bloedingen na tonsillectomie zijn een bekend risico en ingeslikt bloed kan een verklaring zijn voor de genoemde postoperatieve misselijkheid en overgeven.6,7) Pijn na tonsillectomie lijkt moeilijk op te lossen8-10), ondanks onderzoek naar de effecten van: (i) pre- en postoperatieve medicatie7,8), (ii) lokale anesthetica8,13), (iii) chirurgische technieken14), (iv) antibiotica15), (v) postoperatief pijnmanagement2,16-19) en nonfarmacologische interventies20,21). Langdurig preoperatief vasten lijkt samen te hangen met postoperatieve misselijkheid22), terwijl kortdurend vasten de kans op misselijkheid lijkt te verminderen en aspiratie van de maaginhoud in de longen lijkt te voorkomen18,23). De richtlijnen zijn op basis hiervan bijgesteld.18,23) Kinderen mogen 2 uur voor de operatie drinken en 4 tot 6 uur voor de operatie eten, maar kinderen vasten – ondanks richtlijnen – veelal nog steeds te lang24,25) en het lijkt relevant kinderen en hun ouders beter voor te lichten over gepaste voeding en vochtinname voor de operatie. Naar het oordeel van de auteurs is het effect van preoperatieve voeding op pijn na tonsillectomie nog niet onderzocht. …
