Samenvatting

Het meten van allergeenspecifieke IgE-antistoffen in serum is een belangrijk hulpmiddel in het verzamelen van informatie over de sensibilisatie van een patiënt voor specifieke allergenen. Hoewel de specifieke IgE-metingen de laatste jaren sterk verbeterd zijn, blijven er discrepanties bestaan tussen de concentratie specifiek IgE tegen een bepaald allergeen en de aan- of afwezigheid van klachten na contact met dit allergeen.Veel van deze discrepanties kunnen verklaard worden uit het feit dat een IgE-test een methode is om allergische sensibilisatie aan te tonen maar dat sensibilisatie niet hetzelfde is als klinische reactiviteit. Daarnaast wordt in een specifieke IgE-bepaling regelmatig IgE aangetoond die niet (of nauwelijks) betrokken is bij een allergische reactie. Dit laatste is vooral het geval bij voedselallergie. Bij een verdenkingvan voedselallergie kan daarom in veel gevallen de specifieke IgEtest een voedselprovocatie nog niet vervangen.
