A 76-year-old lady was admitted to the cardiology department with acute typical chest discomfort and dyspnoea. The patient had a medical history of diabetes mellitus type 2, hypercholesterolaemia and chronic obstructive pulmonary disease. Furthermore, she smoked 12 cigarettes a day. The patient had no history of heart disease. On physical examination no abnormalities were observed. Laboratory tests revealed a troponin-I of 4.91 μg/L and a CK of 212 U/L with a peak value of 660 U/L within 24 h after admission. The 12-lead ECG (Fig. 1) showed a sinus rhythm with a rate of 69 beats/min, normal conduction times and an isolated ST-segment elevation in V2. What is the diagnosis?×
