Rupturen van de peroneusbrevispees. Anatomie, pathofysiologie, therapeutische interventies en resultaten

Samenvatting

Laterale enkelpijn is een veelvoorkomende klacht, met een breed scala aan oorzaken. Rupturen van de peroneusbrevispees (PBP) worden vaak ondergediagnosticeerd. Gericht onderzoek naar eventuele zwakte, pijn en zwelling van de PBP is van essentieel belang om tot een goede (chirurgische) behandeling te komen. Aangezien een ruptuur van de PBP gebaseerd is op een mechanische abnormaliteit, is chirurgische interventie vaak geïndiceerd en moet de onderliggende oorzaak van de ruptuur behandeld worden. Anatomische variaties kunnen hierbij ook als oorzaak gezien worden. Zowel echografie, mits in ervaren handen, als MRI is nuttig bij het opsporen van pathologie.
PBP-rupturen verlopen vaak longitudinaal, treden op aan het distale uiteinde van de fibula en kunnen goed chirurgisch behandeld worden, afhankelijk van de lengte van de scheur en de kwaliteit van de pees. Er zal dan ofwel gekozen worden voor het tubulariseren van de pees, dan wel voor het verrichten van een versterkingsplastiek. Voor een zo goed mogelijk klinisch resultaat is het van belang dit ‘complex aan afwijkingen’ in zijn geheel te behandelen. Studies suggereren dat vroege diagnose en chirurgische behandeling leiden tot het op termijn pijnvrij functioneren van patiënten, zelfs bij atleten.
De oorzaak van PBP-rupturen is vaak gelegen in een combinatie van microvasculaire, mechanische en anatomische factoren. Gerandomiseerde prospectieve studies zijn nodig om de optimale therapie bij PBP-rupturen vast te stellen.
