Samenvatting

Voorgeschiedenis, anamnese en gericht lichamelijk onderzoek zijn belangrijke hulpmiddelen bij de diagnostiek van een rood oog. Conjunctivitiden komen verreweg het meest voor. Iritis en scleritis zijn geassocieerd met systeemziekten. Een ernstig trauma van de oogkas kan leiden tot een orbitabodemfractuur met inklemming van oogspieren en een contusie of perforatie van de oogbol. Pijn, lichtschuwheid, misselijkheid/braken, verminderde visus en pupilafwijkingen wijzen op een ernstige oogaandoening, bijvoorbeeld een virale of bacteriële conjunctivitis. Jeuk is het belangrijkste kenmerk van allergische conjunctivitis. Pathologie van de cornea, zoals keratitis en het cornea-ulcus, gaat gepaard met pericorneale roodheid en een vermindering van het gezichtsvermogen. Kenmerkende bevindingen bij iridocyclitis zijn pericorneale of gemengde roodheid, een eenzijdig nauwe of vervormde pupil, een pijnlijke consensuele lichtreactie en visusvermindering. Ernstige pijn, braken, een middelwijde, lichtstijve pupil en een verhoogde oogboldruk wijzen op acuut glaucoom. Met fluoresceïne kan men erosies en ulcera van de cornea aankleuren. Spleetlamponderzoek kan kleine corpora aliena, lokale ontstekingsprocessen en afwijkingen aan het licht brengen, bijvoorbeeld een microperforatie.
