13. Hoofdpijn

Samenvatting

Hoofdpijn is een veelvoorkomende klacht met voor de patiënt vaak hinderlijke consequenties. Aandacht voor de reden van de komst, bijvoorbeeld angst voor een ernstige aandoening, en de verwachtingen van de patiënt is van groot belang. Primaire hoofdpijnvormen zijn: spanningshoofdpijn, migraine en clusterhoofdpijn. Daarnaast is het van belang om alert te zijn op mogelijke ernstige oorzaken van (secundaire) hoofdpijn, hoewel deze zeldzaam zijn. Bij de diagnostiek is een zorgvuldig afgenomen anamnese het belangrijkste. De bijdrage van het lichamelijk onderzoek is meestal zeer beperkt. Aanvullend onderzoek of verwijzing is alleen zinvol als bij secundaire hoofdpijn aan een ernstige oorzaak wordt gedacht. Het indelen van hoofdpijn in een diagnostische classificatie is bij kortdurende klachten niet altijd mogelijk. Bij langer durende hoofdpijn is het belangrijk attent te zijn op de mogelijkheid dat verschillende vormen tegelijkertijd kunnen voorkomen.
