Resultaten van fractuurstabilisatie met Less Invasive Stabilization System bij fracturen van distale femur en proximale tibia

Samenvatting

Doel. Klinische en radiologische evaluatie van het gebruik van het Less Invasive Stabilization System (liss) bij periarticulaire fracturen van de knie. Opzet. Retrospectieve analyse van patiënten die geopereerd zijn met een liss in de periode van 2002 tot en met 2006.
Patiënten en methoden.Uit 133 ossale knieletsels, waarvan 52 fracturen werden behandeld met het liss, zijn 32 patiënten (34 fracturen) geanalyseerd met een minimale follow-up van 6 maanden. 15 Patiënten hadden een distale femurfractuur, 19 een proximale tibiafractuur. De mediane leeftijd was 74 jaar (21-92 jaar). De repositie en stabiliteit werden bepaald door middel van de postoperatieve röntgenonderzoeken. De functie werd bepaald met de Oxford Knee Score (oks).
Resultaten.De gemiddelde follow-up was 19 maanden (6-39 maanden). 1 Patiënt ontwikkelde een diepe wondinfectie, waarvoor het liss uiteindelijk moest worden verwijderd. 1 Patiënt ontwikkelde diepe veneuze trombose. Bij 4 patiënten werd osteosynthesemateriaal verwijderd vanwege persisterende pijnklachten. Bij 27 patiënten werd de oks afgenomen, gemiddeld 19 maanden (6-39 maanden) na operatie. De gemiddelde score bedroeg 32 (12-54).
Bij 5 patiënten werd een postoperatieve standsafwijking gezien, bij allen zonder klinische consequenties. Bij bijna alle patiënten zorgde het liss voor voldoende stabiliteit van de fractuur tijdens fractuurgenezing.
Conclusie.Stabilisatie van periarticulaire fracturen van de knie met het liss geeft onzes inziens een acceptabel complicatierisico dat vergelijkbaar is met de literatuur. De stabilisatie is toerijkend om de reductie te behouden, en postoperatieve pijn en functie zijn eveneens acceptabel.
