Heupfracturen bij bejaarden

Samenvatting

De heupfractuur bij de bejaarde patiënt kan worden gezien als de epidemie van de 21^e eeuw. De incidentie is hoog en neemt jaarlijks toe. Niet alleen de fractuur op zich, maar zeker ook de algemene conditie van de patiënt en zijn/haar pre-existente functioneringsniveau is van groot belang voor de prognose. De mortaliteit na een jaar is hoog (30%) en een substantieel deel van de patiënten bereikt niet meer het oude niveau.

Fractuurclassificatie is belangrijk voor het kiezen van de optimale behande-lingstechniek op chirurgisch gebied, waarin het profiel van de patiënt eveneens in belangrijke mate meetelt. Vroege anticipatie op revalidatie en resocialisatie in een multidisciplinaire ketenzorg is noodzakelijk. Algemeen lichamelijke aspecten, maar ook de psychosociale condities spelen hierbij een grote rol. Essentieel in de chirurgische behandeling is in het geval van een osteosynthese het verkrijgen van een optimale repositie, zo nodig met een open techniek. De aspecten van het toegepaste implantaat zijn eveneens als kritisch te beschouwen in relatie tot de kans op slagen of falen van de operatie.

Wat betreft de langetermijnresultaten dient aansluiting te worden gezocht op richtlijnen voor osteoporose en valpreventie.
