Respiratoir syncytieel virus bronchiolitis: rol van genetische polymorfismen

Samenvatting

De variabiliteit in ernst van ziekte na infectie met het respiratoir syncytieel virus (rsv), alsook het risico op recurrent piepen in de jaren volgend op de infectieuze episode, wordt beïnvloed door een aantal epidemiologische en demografische factoren (leeftijd, maand van geboorte, roken in het gezin), en de klinische voorgeschiedenis van de patiënt (prematuriteit, chronisch longlijden, congenitaal hartlijden). Daarnaast bepalen polymorfismen in genen coderend voor belangrijke eiwitten van het immuunsysteem wellicht de uitkomst van de infectie bij een bepaald individu. Een aantal van deze genetische polymorfismen is inmiddels beschreven en geassocieerd met ernstige ziekte na infectie met rsv. Helaas is de functionele betekenis van de meeste polymorfismen nog onduidelijk, zodat de bruikbaarheid als prognostische factoren in de kliniek vooralsnog gering is.
