Samenvatting

Het prikkelbaredarmsyndroom (PDS) betreft een wereldwijd frequent voorkomend klachtencomplex van buikpijn in relatie tot verandering in ontlasting. Zowel in de epidemiologie als in het beleid tekent zich een verschil af tussen mannen en vrouwen. De pathofysiologie van deze genderspecificiteit wordt geleidelijk duidelijker en is waarschijnlijk in belangrijke mate terug te voeren op gendergerelateerde neuro-endocriene processen. De behandelaar zal bij anamnese en onderzoek rekening houden met deze achtergrond. Voor de behandeling van PDS is onderscheid tussen mannen en vrouwen nog onvoldoende onderbouwd. In de afgelopen jaren is veel onderzoek verricht naar dieetmaatregelen, vooral het FODMAP-dieet. Daarnaast is bewijs gekomen voor de effectiviteit van hypnotherapie. Geneesmiddelen hebben een bescheiden plaats in de behandeling, maar er zijn sterke aanwijzingen dat farmacotherapie met 5-hydroxytryptamine-3-receptorantagonisten een gendergerichte aanpak vraagt.
