Samenvatting

Pijn op de borst komt vaak voor en in driekwart van de gevallen gaat het om relatief onschuldige aandoeningen. Niettemin is uitsluiten van een ernstige aandoening – zoals acuut coronair syndroom (ACS), longembolie, aortadissectie of pneumothorax – van groot belang. Bij pijn op de borst met een stabiel en voorspelbaar karakter ligt het accent vooral op het aantonen of uitsluiten van onderliggend stabiel coronairlijden. De anamnese is bij de beoordeling van pijn op de borst het belangrijkste instrument. Denk hierbij aan de aard van de pijn en de bijkomende symptomen, het ‘niet-pluis gevoel’, de aanwezigheid van risicofactoren en de medische en familiaire voorgeschiedenis. Lichamelijk onderzoek is van aanvullende waarde bij bevindingen die passen bij een tendomyogene of pulmonale oorzaak. Het maken van een elektrocardiogram is geïndiceerd bij een verdenking op een ritmestoornis of hartfalen. De Wells-beslisregel in combinatie met een D-dimeer wordt gebruikt voor het uitsluiten van een longembolie. Bij een verdenking op een ACS blijft spoedverwijzing de enige optie.
