Samenvatting

Hoewel hoesten op zichzelf een nuttig en zelfs noodzakelijk verschijnsel is, kan het door de frequentie of ernst zo hinderlijk worden dat hiervoor medische hulp wordt gezocht. Het is onjuist te veronderstellen dat alle hoestklachten op een infectie berusten. Vooral bij langer durende klachten is het zinvol de diagnostiek verder uit te breiden en aandacht te besteden aan de aanwezigheid van astma, hyperreactiviteit, chronic obstructive pulmonary disease (COPD), hartfalen of allergische rinitis. Bij ouderen en forse rokers is het van belang een longcarcinoom in de differentiële diagnose op te nemen. Andere aandoeningen die het hoesten kunnen veroorzaken zijn luchtweginfecties, (acute/chronische) bronchitis, bronchiolitis, pneumonie, gastro-oesofageale reflux, longfibrose en cystische fibrose (mucoviscidose). De diagnose bij chronisch hoesten kan in een groot aantal gevallen worden gesteld op basis van anamnese en lichamelijk onderzoek. Longfunctieonderzoek kan zinvol zijn omdat het inzicht geeft in de ernst van de obstructie en de prognose. Complex aanvullend onderzoek is zelden noodzakelijk en alleen zinvol bij verdenking op een ernstige infectie, interstitiële longafwijkingen, hartfalen of een maligniteit als oorzaak van het hoesten.
