Open tibiafracturen bij kinderen

Samenvatting

Open fracturen van het onderbeen bij kinderen dienen net als bij volwassenen agressief te worden behandeld. Preventie van infectie is een eerste vereiste in de behandeling van open fracturen. De eerste behandeling bestaat dan ook in ieder geval uit een uitgebreid debridement en irrigatie. In dezelfde ingreep dient de fractuur te worden gestabiliseerd. De wond dient primair dan wel secundair te worden gesloten, afhankelijk van de ernst van het letsel. Het gebruik van een externe fixateur als definitieve fixatie is geassocieerd met infectie en delayed union. Daarom adviseren wij deze fixatiemethode voornamelijk te gebruiken als damage control bij de polytraumapatiënten. Een mogelijke uitzondering is de open fractuur graad IIIB en C volgens Gustillo, waarbij de externe fixateur in specifieke gevallen als definitieve fixatie kan dienen.
Een goede wekedelenbedekking van de fractuur moet het liefst direct worden gerealiseerd, maar in ieder geval binnen zeven dagen. Dit leidt tot een afname van het aantal complicaties. Kleine wonden zonder spanning kunnen primair worden gesloten, of men kan kiezen voor een primair vrij huidtransplantaat. Is dit niet mogelijk, bij open graad II- of III-fracturen, dan kan het defect – meestal in een tweede tempo – met behulp van een lokale transpositie of met een vrije lap worden bedekt.
