NIV-richtlijn: Zwangerschap bij diabetes

Inleiding Een herziene CBO/NIV-richtlijn beantwoordt de vraag of screenen op zwangerschapsdiabetes mellitus zinvol is.

Gevolgen van zwangerschapsdiabetes Zwangerschapsdiabetes geeft een verhoogd risico op macrosomie. Dit leidt weer vaker tot schouderdystocie, fracturen en plexusbrachialisbeschadigingen. In zeldzame gevallen komt perinatale sterfte voor.

Invloed van behandeling Systematische reviews maken niet duidelijk of behandeling de perinatale morbiditeit en mortaliteit vermindert. Een groot gerandomiseerd onderzoek toonde een verminderd relatief risico aan van 0,33 (95%-BI 1,15-1,62) op perinatale sterfte, schouderdystocie, een humerusfractuur of plexusbrachialisbeschadiging.

Welke screeningstests? De sensitiviteit en specificiteit van screeningstests is matig. De diagnostische (orale) GTT (de ‘gouden standaard’) is slechts beperkt reproduceerbaar.

Is screening zinvol? Screening in de tweede helft van de zwangerschap wordt niet aanbevolen. Bij symptomen en klachten die duiden op zwangerschapsdiabetes, is een diagnostische GTT nodig.

De richtlijn geeft ter overweging om aan het begin van de zwangerschap te screenen op pre-existente, nog niet eerder ontdekte diabetes met een nuchtere glucosebepaling. Het beleid sluit aan bij het opsporen van diabetes buiten de zwangerschap zoals beschreven in de NHG-Standaard Diabetes mellitus type 2.
