7. Neurofysiologie van pijn

Samenvatting

Pijn is een multidimensionaal fenomeen met zowel sensorische als emotionele aspecten. Het leidt onherroepelijk tot gedragsveranderingen bij hen die eraan lijden. Deze veranderingen zijn erop gericht om verdere schade aan weefsels te voorkomen. Het pijn-adaptatiemodel beschrijft hoe dat in zijn werk gaat in geval van bijvoorbeeld een pijnlijk kaakgewricht, waarbij agonistisch werkende spieren zullen inhiberen en antagonisten zullen exciteren. Een ander model voor pijn is de poorttheorie waarmee een verklaring wordt gegeven voor het feit dan pijn moduleerbaar is, nog voordat er pijnperceptie plaatsvindt. Het meten van de uiteindelijk ervaren pijn kan op verschillende manieren. Welk instrument te kiezen, hangt af van de setting en het soort patiënt dat wordt onderzocht.
