11. Manie en bipolaire stemmingsstoornissen

Samenvatting

Manische toestanden zijn meestal onderdeel van bipolaire stemmingsstoornissen, een aandoening die vaak al in de jonge volwassenheid begint en nadien in de meeste gevallen recidiveert. Een manie verloopt meestal heftig, met forse gedragsafwijkingen, gebrekkig ziektebesef, en soms psychotische verschijnselen, en wordt vaak gevolgd door een diepe depressie. De emotionele en maatschappelijke gevolgen kunnen achteraf groot blijken te zijn. Bij een welbespraakte patiënt wordt de ernst van de manie vaak onderschat. Mensen in de omgeving zijn eerder gealarmeerd dan de patiënt zelf, en dienen dus altijd bij de behandeling betrokken te worden. De multidisciplinaire behandeling is primair medicamenteus, naast interventies om het ongeremde gedrag te begrenzen en de patiënt te beschermen tegen zichzelf. Vaak is een al dan niet vrijwillige opname nodig. De afgrenzing van schizoaffectieve psychotische stoornissen is onscherp. Ook de manische episode die nadien volledig in remissie gaat, dient opgevat te worden als een EPA, met een indicatie voor langdurige onderhoudsbehandeling.
