Abstract

De Nederlandse bevolking vergrijst en heeft te maken met een toename van patiënten met een of meer chronische ziekten, zoals diabetes, hartfalen, nierziekten of dementie (Figuur 1). Deze demografische ontwikkeling, waarbij er meer ouderen zijn en minder jongeren, leidt tot een toename in de zorgvraag over een langere levensperiode en een afname in de potentiële arbeidskracht in de zorg.¹ De verwachting is dan ook dat in 2025 een op de drie patiënten geen adequate toegang heeft tot professionele zorg.²
