Samenvatting

Hoewel Antoni van Leeuwenhoek reeds in 1683 de aanwezigheid van tandplaque en de daarin aanwezige bacteriën beschreef, duurde het tot in deze eeuw voordat men het belang van tandplaque bij het cariësproces inzag.

Miller, een Amerikaanse tandarts, werkte in het laboratorium van Koch in Berlijn. Via een serie eenvoudige maar overtuigende experimenten toonde hij aan dat de orale flora via fermentatie van koolhydraten glazuur- en dentinecariës kan veroorzaken. Melkzuur werd in voldoende hoeveelheid uit bijvoorbeeld brood of suiker geproduceerd om ontkalkingen van een in speeksel geïncubeerde tand te veroorzaken. In zijn boek uit 1889 Die Mikroorganismen der Mundhöhle vat hij zijn cariëstheorie samen: ‘Cariës is een chemo-parasitair proces dat ontkalking of verweking van de harde weefsels bewerkstelligt. De verweekte weefsels gaan vervolgens in oplossing, waarbij een caviteit wordt gevormd: het glazuur lost direct op door de zuren die bij de bacteriële fermentatie vrijkomen, het door deze zuren verweekte dentine wordt ook nog afgebroken door proteolytische enzymen.’ Hoewel Millers chemo-parasitaire theorie in principe nog houdbaar is, verklaart zij niet waarom cariës op lokale, specifieke plaatsen op de tand optreedt. Miller werkte met gemengde culturen van bacteriën uit het speeksel. De theorie verklaart niet waarom sommige mensen cariësvrij zijn, noemt nauwelijks specifieke bacteriën en gaat niet in op de rol van tandplaque.
