54. Lage rugpijn

Samenvatting

Lage rugpijn komt in de algemene bevolking zeer veel voor, heeft meestal ( > 90 %) geen specifieke oorzaak en herstelt vaak weer binnen enkele weken. Een klein aantal mensen heeft chronische pijnklachten (meer dan 3 maanden), ervaart frequent recidieven en heeft langdurig functionele beperkingen. Deze relatief kleine groep genereert hoge maatschappelijke kosten, vooral door langdurig ziekteverzuim en blijvende arbeidsongeschiktheid. Mogelijk specifieke oorzaken zijn lumbosacraal radiculair syndroom (o.a. hernia nuclei pulposi ofwel ischias, wortelkanaalstenose/laterale stenose, caudasyndroom), anatomische afwijkingen (osteoporotische inzakkingsfractuur, maligniteit, spondylolysis/spondylolisthesis) en de ziekte van Bechterew ofwel spondylitis ankylopoetica. Bij de diagnostiek van lage rugpijn moet de arts enerzijds specifieke oorzaken detecteren, anderzijds moet hij bij aspecifieke lage rugpijn alert zijn op dreigende chroniciteit en disfunctioneren. Meestal volstaan een goede anamnese en zorgvuldig lichamelijk onderzoek. Het doen van aanvullend of specialistisch onderzoek is voornamelijk aangewezen wanneer er aanwijzingen zijn voor een specifieke oorzaak van de rugpijn.
