6. Kortetermijncomplicaties na chirurgisch geassisteerde snelle palatinale verbreding

Samenvatting

Een transversale hypoplasie van de maxilla is een van de meest voorkomende skeletale afwijkingen in de tandartsenpraktijk. Doordat meer volwassenen een orthodontische correctie wensen, zien we ook een toename in de chirurgisch geassisteerde verbreding van het verhemelte. Deze ingreep berust op het losmaken van de groeinaden in de maxilla, waarna osteodistractie volgt door middel van een element- of botgedragen expansieapparaat. Hoewel deze techniek relatief weinig complicaties kent in vergelijking met orthognathische heelkunde, worden bij bijna de helft van de patiënten één of meerdere complicaties vastgesteld. Naast de klassieke verschijnselen na de operatie, zoals pijn, nabloeden en een afgenomen functionaliteit van het kauwapparaat, worden voornamelijk zenuwschade van de nervus infraorbitalis, dentale problemen, waaronder tandmobiliteit en asymmetrische of inadequate expansie, genoemd. Verder worden ook enkele heelkundige overwegingen en contra-indicaties toegelicht. We besluiten met de constatering dat alleen goede communicatie met zorgvuldige indicatiestelling en patiëntenselectie kunnen leiden tot een in zijn totaliteit geslaagde behandeling.
