7. De rol van condylaire veranderingen in de botstabiliteit na verlengingsosteotomie van de onderkaak (BSSO)

Samenvatting

Bilaterale sagittale splijtingsosteotomie (BSSO) is de meest uitgevoerde operatieve ingreep om de mandibula te verlengen bij patiënten met mandibulaire hypoplasie. Na de BSSO wordt vrijwel altijd enige terugval (relapse) in kaakstand gezien. Skeletale relapse kan zowel de esthetiek als functie negatief beïnvloeden. Een afname van het volume van de kaakkopjes na een BSSO is geassocieerd met het ontstaan van skeletale relapse. Door het vervaardigen van een cone beam computertomografie (CBCT) voorafgaande aan de BSSO en het bepalen van het condylair volume kan het risico op relapse beter worden ingeschat dan op een orthopantomogram of lateraal cefalogram. Er zijn aanwijzingen dat biochemische factoren zoals de vrouwelijke geslachtshormonen en botdichtheid ook een rol spelen in het ontstaan van postoperatief condylair volumeverlies en skeletale relapse.
